How to get and use Sacrificial Heart in Path of Exile 2

Path of Exile 2 features multiple hidden quest items to reward players, one of which is the Sacrificial Heart. It’s a giant heart that you can sacrifice at a specific altar to complete an unlisted side-quest during the campaign. While doing so isn’t necessary, the two passive skill points you get as a reward are worthwhile. Besides, finding the item won’t be challenging even if you’re trying to speedrun.

This guide will show you where to find the sacrificial heart and how to use it to complete the secret side quest ‘Blood Sacrifice’ for passive skill and weapon set points.

How to find Sacrificial Heart in Path of Exile 2

The Sacrificial Heart is limited to enemies during Act 3 in Path of Exile 2, but there’s a limitation on who and in which area this item drops from. Since it’s a giant heart, you’ll eventually get it by defeating Vaal Goliaths, the biggest one among regular enemies.

After you enter the Temple of Kopec to defeat Ketzuli and unlock the time machine, go back to Utzaal in time. Follow the main quest and make your way to the area boss, Viper Napuatzi. By this time, the Vaal Goliaths should drop the heart.

The Sacrificial Heart will make a unique sound on drop and will have green text. However, don’t worry if you can't find it by defeating Goliaths Utzaal, the next area, Aggorat, can also drop the item.

To get your rewards, you’ll need to offer the Sacrificial Heart at an altar, because Atziri requires sacrifice in exchange for power. After picking it up, look for the altar in the Aggorat area. You may need to explore the entire map if you’re unfamiliar with the layout, but it's generally on the right side in the morgue-like area called Trial of Sacrifice.

There’ll also be a checkpoint near the altar in case you find Doryani’s lab entrance first. Some of the Vaal followers will already be conducting a sacrifice. Defeat them and pick up the Sacrificial Dagger, then place the heart on the altar and use the dagger to stab it and complete the quest.
