You can trigger Hidden Ugliness once you reach Flower Expanse in the Qinghe region. When you arrive, look for a group of children gathered outside Lady Ugly’s house.

Get close enough to hear them talking, and the quest will activate automatically.

Confront the children and meet Lady Ugly

Walk up to the kids and interact with them to scold their behavior. After they scatter, head inside the house and speak with Lady Ugly for the first time. This conversation sets the tone and sends you into the next section.

Walk through the Flower Expanse

Lady Ugly asks you to take a walk with her. Stay beside her as she slowly moves through the field. She’ll eventually stop to admire a bright red flower. Once she finishes, the walk concludes.

Search for the Water Lady’s letter

Return to the house and look for the letter mentioned during your conversation. It’s lying on a table near the front window. Read it to continue.

Get help from the children and catch three Mutant Golden Mantises

Step outside and talk to the kids again. They’ll reluctantly help you by pointing out Mutant Golden Mantises. These creatures are tiny and easy to miss, so rely on the quest markers.

You must observe each mantis, then chase it down to capture it. Do this three times.

Tell Lady Ugly about the riddle then accept her 'treat'

Go back to Lady Ugly’s home and update her about what you learned. She invites you inside for flower cakes along with the children.

After eating, your character collapses. When you wake up, the house is empty. Activate Wind Sense to figure out what happened.

Follow the footprints out of the house

Wind Sense reveals a trail leading away from the door. Follow the footprints until they end near a roadside lamp. Investigate the end point, and the game sends you straight to Evercare Clinic.

Investigate the clinic and head toward the waterfall

Inside Evercare Clinic, check the Open Medical Manual lying behind Unfated Tian. Then use Wind Sense again to pick up the new footprint trail.

Follow it all the way to a waterfall. Step inside the falls to trigger another encounter with Lady Ugly.

Wake up in Evercare Clinic and speak with Yao Yaoyao

After the waterfall confrontation, you awaken in the clinic. Talk to Yao Yaoyao, who sends you to check on the three children.

Visit the children's families and return to Lady Ugly’s resting place

Head to Harvestfall Village and speak to each child’s family. Once done, travel back to Flower Expanse. Behind Lady Ugly’s home, next to the same red flower from earlier, you’ll find her grave. Pick up the Disguise Balm placed there and pay your respects.

Make your final choice

Interact with the grave again. Here, you choose what to do with the Disguise Balm and decide the children’s future involvement. This changes some dialogue but does not affect the rewards.
