Anomaly bosses in Path of Exile 2 are special boss fights introduced in patch 0.3. They drop some exclusive Lineage Support Gems in PoE2 that can make your builds quite strong in the endgame. As of now, there are four Anomaly bosses in the game, and all of them are based on the final bosses of Act 4 and all three Interludes.
Since they are part of the endgame, you’ll need to complete the campaign and explore the Atlas. This guide will teach you everything you need to know about Anamoly bosses in Path of Exile 2, including how to find them and a list of their exclusive rewards.
All Anomaly bosses in Path of Exile 2
1) Manoki, the Chosen
Jade Isle (Image via GGG)Manoki is the final boss of the Jade Isle map in the endgame. He is based on the Karui Chieftain, Tavakai, from Act 4 in Path of Exile 2. However, despite being a part of the endgame Atlas, the map is not accessible right away. On top of being a rare map, it is shrouded by a Maelstrom that you need to remove first.
To find the island, explore the Atlas near water areas. The Maelstrom is easy to spot, and once you do, you’ll need to clear the connected maps and disable a Karui Beacon. Upon entering, you’ll find the layout similar to the Nagakanu island after Tavakai steals the ancient weapon and afflicts everyone with Blood Fever.
The fight itself is the same as the Act 4 showdown and takes place across three phases. Here’s a quick recap of what to expect:
- Phase one: You’ll face Manoki (Tavakai) in semi-human form, but after taking down his HP by 60%, he’ll transform and run away near Tukohama’s shrine. During the process, he’ll regain all of his health. Watch out for a slam attack during this fight.
- Phase two: During this phase, Manoki will start using corrupted attacks alongside some powerful slams. Many of his attacks include bringing up spikes from the ground. There is a slight delay that gives you a little time to dodge. After bringing his HP down around 80%, he’ll transform once again and reach full HP.
- Phase three: During this phase, the boss will become a lot faster and use many more AoE attacks. If the fight drags on too long, he’ll cover the entire arena with a DoT ground attack that will soon use up your life flask, so defeat him as quickly as possible.

2) Varloch, the Ashen Lord, and Avelyne, the Withered Rose
Derelict Mansion (Image via GGG)Varloch and Avelyne are the boss fights within the Derelict Mansion map node in the Atlas. Both the bosses and the map are based on Path of Exile 2's Interlude 1, The Curse of Holten. To access it, you’ll need to activate Draiocht Hengestones in the connected maps.
You’ll find the map shrouded in a dark aura, with bats flying around the open areas across Atlas. There are also multiple smaller houses around, so it’s a good idea to progress around those maps. The fight is as simple as it can get. Varloch's attack patterns are similar to those of Draven from Act 1, while Avelyne is a modified variant of the Asinia boss fight.
Here’s what you need to look out for during the fight:
- Sword Slam: Varloch will slam his sword in the ground, which you can dodge easily, but there’s a follow-through attack that sends a shockwave on either side of the slam.
- Orbital strike: Avelyne will occasionally form a Bone Cage and call down an orbital strike in the area. Break it quickly and run away.

3) Zahmir, the Blade Sovereign
Temporal Sandstorm (Image via GGG)Zahmir is the boss fight within the Sacred Reservoir map in Path of Exile 2. This one’s based on Azmadi, the Faridun Prince, fighting during the final stage of Interlude 2, The Stolen Barya. While not as annoying as the Manoki fight, Zahmir also has some multi-stage attacks and two different phases.
To find Sacred Reservoir, explore Atlast around the desert area and look for a Temporal Sandstorm. You can remove it by disabling the three Temportal Tethers across all of the connected maps. The map layout is similar to the Qimah Reservoir.
As mentioned, it’s a two-phase fight. Here’s a quick recap of what to expect:
- Phase one: Upon entering the arena, all of the copies will dissolve, and the boss will draw out his sword. Zahmir deals a lot of predictable attacks, but they come at you as a combo, and each one can stagger, keeping you stun locked in a spot. After you lower his HP by 40%, Zarokh, the Temporal, will stop time and juice him up to full HP and additional attack combos.
- Phase two: Not that Zahmir’s supercharged, he’ll perform multiple time-related attacks, which will have a slight delay. You can use it to observe the trajectory and dodge, but a few can take a curve, so be aware. Don’t let the boss off-screen, as you won’t be able to anticipate the attacks.

- Khatal's Rejuvenation
- Zarokh's Refrain

4) Ytzara, Blood Oracle, and Maztli, Flesh-Shaper
Estazunti's Vault (Image via GGG)The Vaal brother-sister duo from Path of Exile 2, Interlude 3, returns as Ytzara and Maztli. You’ll find them in the Estazunti's Vault map within the Atlas, and the best way to spot it is through the massive corruption oozing out of all sides. To find one, explore around the snowy mountains area.
With patch 0.4, you no longer need to find an Idol of Estazunti as a drop in one of the connected maps; instead, it can now spawn as the same floating pillar after defeating the map boss. There are no phases in this fight, but as the HP for each boss goes down, they’ll start using various attacks. If one of them dies, the other is empowered.
Here’s what you need to look out for during the fight:
- Claw attack: One of the most annoying aspects of the fight is the claw attack from the sister. It will leech life and heal her, making the boss encounter difficult. You can dodge her attacks, but if your damage is low, it’ll be hard to survive.
- Corrupted beam: The brother will sacrifice blood to start a laser of corruption that deals significant damage. Luckily, there are huge games, and it’s easy to dodge.
- Goliath Slam: During the fight, the brother can transform into a Vaal Goliath and perform an AoE slam. Stay clear of the blast radius.
Here are the possible rewards for defeating Ytzara and Maztli in Path of Exile 2:
- Atalui's Bloodletting
- Tacati's Ire
- Paquate's Pact
To maximize your chances of getting a rare drop, you can use Precursor Tablets that empower the boss and possess them by a random Azmerian Wisp. Additionally, we recommend using a Tier 15 Waystone with as much item rarity as possible. Do note that losing all revives will result in loss of the map, along with all its rewards.
