Like most unique items, Orsivane can drop from multiple sources, including random mobs and loot chests. However, certain sources offer higher drop rates. Below are the main methods to acquire the unique mace:

1) Boss farming

Farming ladder and uber bosses is the best way to target specific unique items. Orisvane is part of Varshan's loot pool. You can find Varshan in the Malignant Burrow dungeon, located in the Hawezar region (Writhing Mire). Once you defeat him, he will drop his hoard, which guarantees at least one unique item, potentially Orisvane.

To open Varshan's hoard, you will need to gather 12x Malignant Hearts. You can find Malignant Hearts in:
