SXSW 2026: ‘Manhood’ Documentary Exposes Popular New Penis Filler And “Girth Enhancement” Trends

A new documentary has set out to dive into the trend of penis enlargement procedures.

Directed by Daniel Lombroso, Manhood, a 91-minute-long film set to premiere at South by Southwest (SXSW) on Saturday, March 14, “follows Dallas entrepreneur Bill Moore as he attempts to make penis enlargement as commonplace as Botox,” per the festival’s website. The film sees Ruben Ramirez, “a father of five,” and “OnlyFans star” David Smith ” put their bodies—and their insecurities—on the line.”

“Blending dark humor with unexpected empathy, Manhood examines shame, addiction, and the fragile myths of American masculinity,” the film’s description adds.

The film hails from Lombroso and fellow EPs Fenton Bailey, Randy Barbato, Sheila Nevins, Penny Lane, Gabriel Sedgewick, Marcus Gershenberg, Daniel Lombroso, Marissa Heringer, and Cason Crane. Manhood has already caught the attention of the press, as the film is featured on IndieWire‘s list of “15 Films to Check Out” at this year’s festival, as well as on Deadline‘s “SXSW Preview + Hot List.”

According to Variety, the documentary set at the PhaloFILL clinic in Dallas centers on “girth enhancement,” as right now there is no safe manner to add length to a penis.

Per Variety, Lombroso, who said he “met a megachurch pastor getting his penis enlarged” and “met a border patrol agent who may be an ICE agent” while at the clinic, said that the project “felt like a way to talk about what was happening in our country and a lot of the pain that men feel.” As for what lies at the root of the trend, Lombroso said that outside of porn, the rise of the “manosphere” may be a reason why men are seeking out these procedures

According to D Magazine, Moore said that he “wanted people to be able to understand that it’s OK to ask and to talk and do research about this, and have more self-confidence.”

“I really hope this film is going to make it much easier for men to have an augmentation and be comfortable with it and able to talk about it,” he shared.
