Somewhere out there is a person who looked at each of these designs, said “yeah that works,” and approved it. That’s the part that gets me. It’s not that mistakes happen — it’s that multiple people were presumably involved and nobody at any point said “hey, does that sign look like it says something terrible?” There’s a whole chain of command that failed here.
The approval process is broken and these are the evidence.
